I have moved, but did not complete the withdraw form for my
student and need the records sent to the new school.
Have the receiving school send a letterhead fax requesting your previous school’s
student records.
Parent wants to withdraw child.
Go to the school where the student is being withdrawn to sign withdraw papers
so your child’s cumulative file will be processed for the receiving
school. You must provide the new school name and address.
Due to illness or injury, my child will be homebound, how
do I go about having a homebound teacher?
Contact your present school for the homebound coordinator.
